Our Commitment To Your Privacy
360 Talk Therapy is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, notify you following a breach of unsecured PHI, and abide by the terms of the notice currently in effect.
How We May Use And Disclose Your PHI Without Authorization
Treatment
We may use and disclose your PHI to provide, coordinate, or manage your psychiatric and medical care, including consultation with other providers and arranging referrals.
Payment
We may use and disclose your PHI to obtain payment for services, including verifying coverage, submitting claims, and collecting balances due.
Health Care Operations
We may use and disclose your PHI for our internal operations, including quality assessment, provider review, training, licensing, and general administration.

- Most uses and disclosures of psychotherapy notes
- Uses and disclosures for marketing purposes
- Any sale of PHI

You may revoke any authorization in writing at any time, except to the extent we have already relied on it.
Your Rights
- Right to inspect and copy your PHI maintained in a designated record set.
- Right to request an amendment if you believe your PHI is incorrect or incomplete.
- Right to an accounting of certain disclosures we have made.
- Right to request restrictions on certain uses and disclosures. We are not required to agree, except where required by law (e.g., services paid in full out-of-pocket).
- Right to request confidential communications by alternative means or locations.
- Right to a paper copy of this notice, even if you have agreed to receive it electronically.
- Right to be notified of a breach of your unsecured PHI.
Our Duties
We are required to maintain the privacy of your PHI, provide you with this notice, and follow the terms of the notice currently in effect. We reserve the right to change this notice and to make the new notice effective for all PHI we maintain. The current notice will be posted on our website and available at our office.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
