Understanding Your Coverage

Behavioral health benefits vary by insurance plan. Your cost may include a copay, deductible, or coinsurance. We encourage all patients to verify eligibility, behavioral health benefits, and any prior-authorization requirements with our office or their insurer before the first appointment.

In-Network Coverage

We are in-network only with the carriers listed above. We do not provide superbills or bill out-of-network insurance. If your plan is not listed, our cash-pay or sliding-scale options may be a good fit.

  • Photo ID
  • List of current medications and dosages
  • Preferred pharmacy name, address, and phone number
